Dental membership plans

Members receive 15% off the practice’s usual and customary fees for dental procedures, including restorative, cosmetic and elective services. The discount cannot be combined with other offers, promotions or discounts, or applied to contracted or negotiated insurance rates.

Enrollment is available throughout the year. Benefits begin at enrollment and full payment and continue for 12 months.

No. Memberships belong to individual patients, cannot be shared and are valid only at S Dental & Specialties.

Please give at least 48 hours’ cancellation notice. A missed appointment or cancellation within 48 hours forfeits that included appointment. You are responsible for scheduling your visits. Unused appointments are not refunded and do not carry into a new membership year.

The published plan states that memberships do not renew automatically; you may purchase a new membership each year. It also refers to a cancellation notice up to 60 days before renewal. Ask our team to clarify the applicable renewal and notice details when enrolling.

The plan allows a refund during the first two weeks after enrollment only if no membership benefits have been used. Contact the team to discuss cancellation and the terms that apply to your membership.

The published plan permits membership as a supplement to insurance, but the practice does not coordinate the two. Membership discounts cannot be applied to negotiated insurance rates. Ask the office how the terms apply to your care before enrolling.
